itemtype="https://schema.org/BlogPosting" itemprop="blogPost" >

An accurate diagnosis is the foundation of any successful hip treatment. The patient’s individual medical history and the severity of their symptoms are central to the physician’s evaluation. Modern medicine takes into account not only objective findings but is also guided to a large extent by the extent to which the symptoms interfere with the patient’s daily life. An experienced orthopedic surgeon will therefore first ask in detail when and during which activities the pain occurs, how it feels, and what limitations it causes.

The first and most important step toward recovery is a detailed medical history, during which specific questions are asked about the nature, duration, location, and intensity of the pain. Assessing the individual’s level of distress is also of central importance: To what extent does the pain limit you in your daily life, at work, or during sports? This subjective component is often more decisive than X-ray findings alone.

The clinical examination provides crucial clues about the cause of the pain. Through targeted movement tests, pressure point examinations, and functional tests, the specialist can draw important conclusions even during the physical examination. This involves analyzing range of motion, muscle strength, and gait. The rotation test, for example, is particularly informative: if rotating the foot inward triggers hip pain, this often indicates hip osteoarthritis. This degenerative joint disease usually develops gradually and presents with characteristic symptoms. In this process, the protective layer of cartilage between the femoral head and the acetabulum gradually wears away. In advanced stages, bone rubs against bone, leading to pain, stiffness, and limited range of motion.

Typical symptoms initially include morning stiffness that improves after the first few movements, increasing discomfort with activity, and a gradual stiffening of the joint. Later, pain during activity sets in, often manifesting as a deep, pulling pain in the groin that can radiate into the thigh. In the final stage, pain occurs even at rest and at night, which significantly increases the burden of the condition. Mobility decreases, and activities such as tying shoelaces or climbing stairs become a challenge. Contrary to widespread assumptions, however, osteoarthritis is not necessarily a disease of old age—younger people can also be affected, especially following sports injuries or in cases of congenital hip deformities.

Hip impingement (femoroacetabular impingement, FAI) is becoming an increasingly common cause of hip pain among younger, active patients. An abnormality in the shape of the femoral head or the acetabulum causes the bones to rub painfully against each other during certain movements. Symptoms typically occur after prolonged sitting or intense physical activity and can lead to premature cartilage wear over time. Affected individuals often complain of deep groin pain that may radiate to the buttocks or thigh.

Bursitis (trochanteric bursitis) causes characteristic pain on the outside of the hip. Irritation of the bursa leads to localized, severe pain upon pressure at the lateral bony protrusion, the greater trochanter. It often develops due to muscular imbalances or overuse and is particularly noticeable when lying on the affected side. Climbing stairs is also often painful.

Muscular imbalances and tendon issues often play an underestimated role. Our sedentary daily lives, characterized by prolonged sitting, lead to shortening of the hip flexor muscles and weakening of the gluteal muscles. Shortened or weakened muscles around the hip can disrupt joint mechanics and lead to pain caused by overuse—even though the joint itself is healthy. Tendinitis of the adductors or gluteal muscles is also a common cause.

Neurological causes such as sciatica or nerve compression can cause pain to radiate to the hip region. The sciatic nerve can be compressed by herniated discs or muscle tension. A herniated disc or spinal stenosis can cause pain that is mistakenly attributed to the hip.

Avascular necrosis of the femoral head is a rare but serious condition. A disruption in blood flow to the femoral head leads to the death of bone tissue. This often causes sudden, severe pain and, if left untreated, can lead to collapse of the femoral head.

Cause

Typical Symptoms

Those Most Affected

Location of Pain

Hip Osteoarthritis

Pain upon starting to move, stiffness, and later, pain at rest

Older patients

Deep in the groin, radiating into the thigh

Hip Impingement

Groin pain after exercise, pinching

Younger, active people

A stabbing pain in the groin, especially when bending forward

Bursitis

Pain on the side of the hip, tenderness

All age groups

On the side of the hip, at the bony protrusion

Muscular Imbalance

Tension-related pain

Office worker

Variable, often vague

Sciatica

A pulling, electrifying pain

All age groups

Buttocks, radiating down the back of the leg to the foot

Interpreting Specific Types of Pain Correctly

The location of hip pain provides important diagnostic clues. Pain felt deep in the groin usually indicates a problem within the hip joint itself. Typical causes include hip osteoarthritis, hip impingement, or a tear in the labrum.

Pain on the outside of the hip, in the area of the greater trochanter, is more likely to indicate problems with the surrounding soft tissues. The cause is often bursitis or irritation at the tendon insertion points of the gluteal muscles. This distinction is of fundamental importance for treatment planning.

Night pain and pain at rest are symptoms that should be taken particularly seriously, as they often indicate inflammatory processes. They may indicate inflammatory activity in the joint, as occurs in active osteoarthritis, a rheumatic disease, or, more rarely, a joint infection.

Start-up pain is characteristic of degenerative changes in the hip. Before surgery is considered, the full range of conservative treatment options should be exhausted. The primary goal is to relieve pain, improve joint function, and slow the progression of the disease. A multimodal approach that combines various components is most successful in this regard.

Nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or diclofenac have both analgesic and anti-inflammatory effects and are primarily used for short-term relief. For long-term use, they should be applied directly to the skin as a gel or ointment due to potential gastrointestinal side effects. Hyaluronic acid treatments can improve joint lubrication and reduce pain in osteoarthritis, although the effect varies greatly from person to person. Injections directly into the joint—such as cortisone for acute inflammation—can also provide relief.

Physical therapy and targeted exercise therapy are at the heart of conservative hip treatment. Professionally guided exercises can correct muscular imbalances, maintain mobility, and strengthen the muscles that stabilize the joint. It is particularly important to stretch the hip flexor muscles—which are often shortened—and to strengthen the gluteal muscles. Regular physical therapy can not only relieve pain but also slow the progression of degenerative changes.

Physical therapy measures effectively complement active treatment. Heat therapy relaxes tense muscles and promotes blood circulation, while cold therapy helps relieve pain in cases of acute inflammation. Electrotherapy can reach deep muscle layers and influence pain through neurophysiological mechanisms.

Modern therapeutic approaches, such as shock wave therapy and PRP treatments, are becoming increasingly important. Extracorporeal shock wave therapy can provide pain relief for chronic tendon and ligament irritation, while PRP injections—which use platelet-rich plasma—are designed to stimulate the body’s own healing processes. These procedures are still undergoing scientific evaluation but are showing promising results.

The importance of weight loss for overweight patients cannot be overstated. Every kilogram lost reduces the load on the hip joint by a multiple of the body weight during dynamic movements. Physical therapy is not a passive measure, but rather an active form of exercise therapy. Its goal is to correct muscular imbalances that place undue stress on the hip joint. An experienced therapist will first analyze your movement patterns and create a personalized exercise program. Strengthening the gluteal muscles—especially the gluteus medius—improves hip stability and reduces stress on the joint.

Stretching techniques for shortened muscles require patience and consistent practice. The psoas muscle, in particular—the major hip flexor—tends to shorten due to prolonged sitting and can cause significant hip pain. Professionally guided stretching exercises can relieve this shortening and significantly improve mobility.

Gait training and movement optimization help correct harmful movement patterns. Due to pain, many patients develop compensatory movements that can lead to further problems over time. A physical therapy gait analysis can identify and correct such postural issues. Surgery is considered only after all conservative treatment options have been exhausted over a period of at least three to six months and adequate relief has not been achieved.

The key indicator is not the X-ray findings alone, but rather the patient’s subjective distress combined with a significant impairment of function and quality of life. Key factors include pain intensity, walking distance, sleep quality, and the ability to perform daily activities. If nighttime pain disrupts sleep, walking distance is limited to a few hundred meters, and participation in social life is barely possible, surgery may be the best option.

Hip arthroscopy has become established as a minimally invasive alternative for younger patients with hip impingement. Through small incisions, damage to the labrum can be sutured, or bony spurs causing impingement can be removed. The goal is to preserve the joint and prevent future osteoarthritis. Compared to open surgery, arthroscopy offers the advantages of faster rehabilitation and less soft-tissue damage.

Hip replacement remains the standard treatment for advanced osteoarthritis that causes significant suffering. In this procedure, the damaged acetabulum and femoral head are replaced with artificial implants. Modern implants have a lifespan of 15 to 20 years or more. The decision between a partial and a total hip replacement depends on the extent of the damage and the patient’s age. Early mobilization is a key focus of postoperative care.

Modern minimally invasive techniques reduce the burden of surgery and significantly shorten the rehabilitation period. By using muscle-sparing approaches, soft tissue damage can be minimized and postoperative recovery accelerated. This results in less pain after surgery, faster rehabilitation, and smaller scars. Nevertheless, every surgery carries risks that must be carefully weighed against the expected benefits.

In addition, corrective osteotomies may be considered: In cases of certain congenital or acquired misalignments, bones can be surgically cut and realigned to improve joint mechanics and slow down wear and tear. This is a joint-preserving procedure, particularly for younger patients.

The realistic chances of success following hip surgery depend on various factors. While most patients experience significant pain relief and improved function, expectations should remain realistic. High-performance sports are usually no longer possible with a hip replacement, but normal daily activities and moderate physical exercise are still possible.

Surgical Procedures

Advantages

Disadvantages

Arthroscopy

Minimally invasive, quick healing

Only for specific indications

Partial denture

Bone mass is preserved

Limited Indications

Full denture

Reliable Pain Relief

Major surgery

Benefits of a Hip Replacement

Disadvantages and Risks of Total Hip Replacement

Significant and lasting pain relief

General surgical risks such as infection, thrombosis, and embolism

Restoring Mobility and Function

Prostheses may loosen over the years; replacement surgery may be necessary

Improving Quality of Life and Mobility

Possible prosthesis dislocation in the early stages

Enables a return to many sports

Leg-Length Discrepancy After Surgery

The Role of a Second Opinion in Surgical Recommendations

Having surgical recommendations critically reviewed through an independent second opinion is an important patient right. Especially in the case of elective procedures such as hip replacement, patients should take sufficient time to make this decision. An additional assessment by a specialist may reveal alternative treatment options or confirm the original recommendation.

A second, independent specialist can critically review the diagnosis and the recommended treatment. It can originate in the joint itself, in tendons and bursae, in the muscles—or it can stem from the lumbar spine. That is precisely why every effective treatment begins with a precise diagnosis: a detailed consultation, a physical examination, and, when necessary, targeted imaging. Only once the source of the pain is clear can a treatment plan be developed that actually addresses the underlying cause.

Equally important is a second principle that runs throughout this guide: We treat the person, not the X-ray image. Pronounced signs of wear and tear visible on imaging do not necessarily mean severe symptoms, and conversely, relatively minor changes can cause significant distress. Therefore, the key factors in any treatment decision are your pain, your range of motion, and the activities you can no longer perform in your daily life, at work, and during your free time.

As a general rule, the approach to treatment is: conservative treatment before surgery. Physical therapy, targeted muscle strengthening, joint-friendly exercise, adjusting the level of physical activity, and—where appropriate—medication or physical therapy offer many options to explore before surgery is even considered. And if conservative measures prove insufficient over a reasonable period of time, modern procedures—ranging from joint-preserving arthroscopy to joint replacement—now offer good prospects for greater mobility and less pain. No one will make this decision over your head—it is a decision we will reach together, after weighing the benefits, risks, and the timing that works best for you.

What you can take away from this guide: Take persistent hip pain seriously, stay active, and have the cause diagnosed instead of waiting for months. Early diagnosis expands your options—the longer improper strain persists, the more the body compensates, and the more complex the correction becomes. The information in this article is not a substitute for an individual medical examination, but is intended to help you ask the right questions.

If you’d like to have your hip problems evaluated at your leisure, you’re welcome to visit the Orthopedics Center Munich East. We take the time to review your medical history, conduct a thorough examination, and openly discuss with you which steps make sense in your situation—and which do not.
