What is a pinched hip?

In a pinched hip (hip impingement), joint mobility in the hip is restricted due to narrowing. The clinical picture is also called femoroacetabular impingement syndrome (FAIS). In this process, the femoral head of the femur abuts the socket of the hip and damages it over time. If left untreated, this can develop into hip osteoarthritis. Pain in the groin is typical for a pinched hip.

Symptoms of hip impingement

Symptoms of a pinched hip can include pain when sitting for long periods of time or restricted movement in the hip due to behaviors designed to avoid the pain (resting posture, resting movement). In this context, hip impingement can occur at all ages from puberty to adulthood. Although most associate hip impingement with age-related issues, it can also occur in competitive athletes, active people with high hip loads in both professional and personal activities, or people with a genetic predisposition.

Pain in the groin or front of the hip is typical, less commonly in the sides or buttocks. Affected patients usually describe the sensation as stinging and pinched. Discomfort when walking uphill or climbing stairs and when bending the hip (e.g., tying shoes) are also common. Occasionally, there are also “snapping” sensations during movement, as in tendon slip disorder, or a painful limp when walking. In addition, the mobility of the hip joint is often limited in a locked hip, especially with regard to the ability to rotate inward. In order to prevent the development of secondary diseases of the hip, immediate treatment is necessary.

Causes of a jammed hip

The specific cause of a hip impingement can only be determined to a limited extent by the orthopedic specialist, since there are indications of possibly promoting causes for hip impingement, but the exact origin has not been scientifically clarified. Risk factors include repeated accidents, including minor injuries to the hip, childhood disorders of hip maturity (e.g. hip dislocation, hip dysplasia), as well as maldevelopment due to incorrect and excessive loading of the hip and hip joints. This can cause anatomical changes in the joint partners femoral head and acetabulum, as in turn leads to the development of hip impingement.

If, on the other hand, it is a deformity of the hip joint based on osteoarthritis, this suggests the precursor of hip osteoarthritis, which must be differentiated from the jamming hip. The orthopedist examines the individual possible cause accordingly in the careful diagnosis of the hip complaints.

Diagnostics in case of a jammed hip

The diagnosis of hip impingement is made by a combination of history, examination findings from clinical examination, and imaging by the orthopedic specialist. In terms of imaging methods, the main focus is on X-rays and a magnetic resonance imaging (MRI) scan to identify changes in the hip joint that trigger the symptoms in a hip that is jammed.

As a result, the orthopedist will be able to determine the type of hip impingement. Although there is no typical classification as for other diseases, the different forms are now common for a classification of the disease according to its change of shape in the anatomical structures:

CAM (femur shaped like a camshaft or pistol grip),
Pincer (hip socket with the shape of a pincher),
Mixed (mixed form of CAM and Pincer) or
extra-articular impingement.

A great deal of experience is required for the diagnosis of hip impingement in order to distinguish the large number of possible differential diagnoses from the clinical picture of hip impingement. Thus, complaints with similar symptoms include mucosal inflammation of the hip, coxarthrosis, lumbar syndromes, or damage to the articular cartilage and many others that need to be differentiated from the jamming hip. Only through the individually correct diagnosis can the appropriate treatment be planned, which is essential for an improvement of the symptoms and the avoidance of secondary diseases.

Treatment for hip impingement

Various conservative therapies may be considered for the treatment of the entrapped hip, depending on its type and severity. These include drug treatments on the one hand, but also physiotherapies on the other. However, more complex variants and especially CAM shape modification are not suitable for treatment with physiotherapy. Accordingly, the experienced specialist must already find the individually suitable variant among the therapies that are specifically adapted to the patient’s needs during the diagnostic phase.

Surgical treatments may be a better alternative for some variants and can be implemented, for example, by hip arthroscopy to reconstruct the physiological shape of the joint in the presence of bony protrusions and growths. The entrapment situation should be improved by the surgical procedure to prevent further stress on the hip joint and restore natural joint motion. Subsequently, sparing and relief of the joint during a healing phase of several weeks and subsequent follow-up treatment with targeted mobilization until the overall situation is free of symptoms are among the possible strategies for treating the jammed hip.
