What is cruciate ligament surgery?

When diseases or more frequent injuries occur in the knee, the cruciate ligaments may be affected. These are tight collagen fiber strands that form tight structures in fiber bundles. As ligaments accompanying the joint, the anterior and posterior cruciate ligaments connect the simultaneously stable and flexible structure that enables the movements of the knee joint. They are called cruciate ligaments because they appear to cross at the center of the knee joint. Strictly speaking, however, they are not located in the joint but outside it, where they enclose the synovial sac. If surgical interventions are necessary due to damage or a tear of the cruciate ligaments, the surgery used in this case is called cruciate ligament surgery.

Cruciate ligaments and their task at the knee

Together with the collateral ligaments, the anterior and posterior cruciate ligaments hold the knee joint in position. They provide stability and mobility, but also limit the ability to rotate and extend the tibia. Despite the name, the cruciate ligaments do not actually run crosswise, but are merely shown to be crossed in perspective due to their different attachment between the knee, tibia, and thigh. In this case, the anterior cruciate ligament runs diagonally backwards to the top, while the posterior one runs laterally past the anterior cruciate ligament diagonally to the top center. Moreover, not only do the cruciate ligaments cross each other in the corresponding perspective view, but they are also each crossed by the opposite lateral ligament, the so-called collateral ligaments.

Thus, the cruciate ligaments show themselves to be the central guiding element for the stabilization and mobility of the knee joint and, at the same time, make possible the natural roll-slide movement with which the knee can be moved in the first place. If the function of one or both cruciate ligaments is damaged, for example by an injury, not only the combination of mobility and stability suffers, but also the fluidity of movement, resulting in increased pressure on the joint cartilage, which in turn can lead to damage to the knee joint.

What diseases and injuries are treated in cruciate ligament surgery?

Numerous conditions and injuries can occur to the cruciate ligaments and cause the need for cruciate ligament surgery. The cruciate ligament tear in particular is feared by those affected, as the complex system of the knee joint encounters numerous challenges and contingencies in the healing process. In the development of complaints of the cruciate ligaments are mostly involved trauma, for example, in a traffic accident, but more often in the context of sports accidents. Depending on the severity, treatment of ACL injuries involves conservative therapies such as physical therapy, cold therapy, or pain management. As a rule, the patient must refrain from previously practiced sports for several months and take appropriate rest. If these methods of treatment are not sufficient, a surgical intervention is performed with a so-called cruciate ligament reconstruction, which, however, is mostly minimally invasive. Cruciate ligament reconstruction as arthroscopic surgery or conservative surgery is counted among the treatments of cruciate ligament surgery.
